Sustainable Local Communities Health Initiative Proposal: A GE Learning Community Approach

Abstract:

Health disparities continue to affect communities in Union County, New Jersey, particularly in Elizabeth, Rahway, and Roselle, where access to preventive care, chronic disease management, and mental health resources remains uneven. This proposal introduces the Union County FairCare Program, a mobile, community-based initiative designed to support sustainable local communities through ethical, preventive healthcare. FairCare would works in conjunction with Local/Union County hospitals, including RWJUH Rahway, by extending long-term improvement efforts that expand diabetes education, strengthen self‑management programs, and increase cancer screenings directly within community settings. Services include preventive screenings, vaccinations, blood pressure and cholesterol checks, and mental health and substance use support. The design of this proposal is informed by our FYS Anchor Text: The Immortal Life of Henrietta Lacks, which highlights the importance of informed consent, patient rights, and trust between medical institutions and marginalized communities. By combining hospital expertise, ethical education, and neighborhood outreach, FairCare models how collaborative healthcare can reduce disparities while fostering healthier, more informed, and resilient sustainable communities.
